Thread Grafting/new Branch Diameter

I was doing some reading and had a question. When you thread graft (the way I understand it) you take a shoot and bend it down (or up) to the location you have chosen for a new branch. This is where I am curious. What happens when you want to put branches lower on the trunk? The shoot will probably be larger in diameter where it meets the trunk and if you are bringing it down to make a new branch, how do you solve the problem of the new branch being smaller in diameter than branches further up on the trunk?
