the secondary trunk should grow from the main one near it's base (as near as possible). You can create it by selecting a branch which is in the desired position and bend it up to make him become a new trunk. If it doesn't have any lower branch, it will be more difficult. You will have to induce bud back and pray to grow a new bud near the tree's base. It really depends on the species, because there are species really hard to bud back on the old wood. And start the new trunk from a new bud. It may take time.
The second trunk should be smaller in diameter than the main one and it's apex should be lower too. Wire them slightly foward of the oder (not so far from the other) and avoid branches wich cross the other trunk.
