www.natureswaybonsai.com. They may still have a few copies. They don't list the book among the more common books offered on their website - give them a call or drop an email to ask about Modern Bonsai. I didn't think to ask if they had any more... I've enjoyed mine this past week!
I think the largest tree I've seen in the book was listed at 33cm. It's fascinating to see how 'tree like' the very small ones can be - now *there* is some artistry. Image/ideal of tree, in miniature.... My favorites (as usual) are the deciduos, flowering, fruiting, err.... There's lots to like in there.
