I wrote a two part article for the ABS Journal on forest plantings. It covers some of the questions that you asked.
I posted a small excerpt of it to the forum but it was not received well by some here. It was, however, received pretty well by the rest of the bonsai community. If you don't get the Journal and still would like a copy of it then PM me with an email address and I'll send you a copy.
It might answer some of your questions.
