At this point I'd feel something has to be done.
This may sound to be against conventional wisdom but.
I'd root prune it enough to get it out of it root bound state or just pot up to the next size. Nothing major maybe 10-15%.
My luck with doing a top pruning at the same time hasn't been very sucessful IMO. I get too much die back. Prune the top after 4-6 weeks when things have harden off.
